Workers Compensation Appointment

If you would like me to evaluate and treat your injury, you just need to to let your workers compensation case worker know. In majority of the cases, the case worker will call my office directly to assure you are seen as soon as possible. If you need to make the arrangements yourself, please:

1. Contact our worker's compensation liaison, at (402) 354-0760 and provide her with: 
     ☛  Your case worker's name and phone number
     ☛  Your supervisor's name and phone number

2. Schedule an appointment with my front desk at (402) 354-0707. Print and fill out the following forms:   
     ☛  Workers Compensation.pdf
     ☛  History and Physical.pdf 
     
☛  Cerner Patient Registration Information.pdf
     ☛  Privacy Notice.pdf

. Bring any medical records pertaining to your current condition
4. Bring any studies (X-ray, MRI, CT, nerve studies) with reports pertaining to your current condition 
5. Bring a Valid ID

I recognize that your time is valuable, and I make every effort to run on time. Occasionally, patients demand a little more time and cause scheduling delays beyond my control. I sincerely apologize if your appointment runs past your scheduled time.

Please, call my office at (402) 354-0707 during business hours and allow at least 1 day’s notice so that we can offer your appointment time to patients on the waiting list.
